Identification, Classification & Task Suitability

  • Identify the governing hazard assessment, traffic-control/work-zone plan, law/contract/owner requirement or site rule, and the garment requirement for this task/zone.
  • Record the complete garment type/category, performance class and configuration actually shown on the label.
  • Where ANSI/ISEA 107 garments are required, are applicable classification and required label information present and legible; otherwise, are governing specification/site markings present?
  • Does the labeled garment classification meet the documented hazard assessment, traffic-control/work-zone plan or governing task/zone requirement?
  • Are required manufacturer identification, safety-critical classification and care instructions legible?
  • Is garment type suitable for documented visibility/coverage needs, weather, other PPE and the governing work-zone/site requirement?
  • Has the vest been altered, had added logos/patches, been cut or shortened, or combined with other garments/equipment in a way that could reduce required background or retroreflective coverage?

Background Material

  • Is background material free of tears, holes or fraying that compromise required coverage, garment retention or manufacturer/site serviceability criteria?
  • Is background material free of soil/contamination that materially obscures high-visibility panels or cannot be corrected by manufacturer-approved cleaning?
  • Is background material free of fading, bleaching, discoloration or loss of conspicuity requiring action under manufacturer/site criteria?
  • Does the garment remain in its original intended configuration, with required background panels not removed, covered, altered or damaged?
  • Garment condition notes: ID, wearer, defect location, disposition and manufacturer guidance consulted

Retroreflective Material

  • Is required retroreflective material securely attached without peeling, lifting or separation compromising continuity/coverage or manufacturer criteria?
  • Are retroreflective strips substantially intact and unaltered in the original labeled configuration?
  • Is retroreflective material free of cracking, crazing or contamination causing loss of continuity, adhesion, visibility or cleanability under manufacturer criteria?
  • Are all required retroreflective elements present, correctly attached and unobstructed in the original labeled/manufacturer configuration?
  • Is required retroreflective material free of removal, shortening, covering or damage making continued classification uncertain?
  • Retroreflective material observations and manufacturer/site evaluation reference

Closures & Wearer Fit

  • Do closures function and secure the garment in its intended position when configured as the manufacturer specifies?
  • Does the assigned garment meet manufacturer sizing and task-specific fit criteria for secure fit, movement, intended coverage and no identified snag/entanglement hazard?
  • In the intended worn configuration, are required high-visibility elements unobscured by closures, outer layers, harnesses, tools or other equipment?

Care & Retirement

  • Has manufacturer-approved care been followed, including any wash-cycle limits, prohibited chemicals and drying restrictions?
  • Are storage practices consistent with manufacturer instructions?
  • Does the garment meet retirement criteria based on manufacturer instructions, relevant laundering history, condition, contamination, alterations and identified site replacement policy?
  • Are damaged, unsuitable or failed garments removed from service, with a suitable replacement provided before exposure continues?

Final Garment Disposition

  • Record final garment disposition
  • Number of vests failing this inspection

Program Verification/Closeout
