Confined Space Entry Permit Checklist
What is a confined space entry permit? A confined space entry permit is a written authorization required by OSHA 1910.146 before workers enter a permit-required confined space (PRCS). Confined spaces cause approximately 90 deaths annually. This checklist ensures proper hazard evaluation, atmospheric testing, ventilation, rescue planning, and worker protection for entries into tanks, vessels, silos, pits, and other enclosed areas.

Confined Space Hazard Identification
Identify all potential hazards per OSHA 1910.146(c)(4) and 1926.1203.
- Atmospheric hazards possible (toxic, flammable, O2 deficient)?
- Engulfment hazard (liquids, granular materials)?
- Mechanical hazards (moving parts, augers)?
- Electrical hazards present?
- Thermal hazards (hot/cold surfaces, steam)?
- Configuration hazards (inwardly converging walls)?
- Other serious hazards identified?
- All hazards documented in space evaluation?
Isolation & Lockout/Tagout
Per OSHA 1910.146(c)(5), isolate the space from all hazardous energy and materials.
- Electrical sources locked out/tagged out?
- Mechanical equipment locked out?
- Piping systems blanked, blinded, or disconnected?
- Steam lines isolated and depressurized?
- Floor drains secured/plugged?
- Adjacent confined spaces isolated?
- All isolation points verified effective?

Atmospheric Testing
Per OSHA 1910.146(c)(5)(ii), test atmosphere before entry and continuously during occupancy.
