Upright & Baseplate Damage Assessment
ANSI MH16.1-2021 (Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks) §8.4 establishes damage assessment criteria and triggers requiring load removal. ANSI MH16.1-2021 supersedes prior editions and is the current standard for industrial steel pallet racking. MH28.3 governs shelving - not industrial pallet racking.
- Each upright column inspected for dents or bends: dents or bends in the column section within 48 inches of floor level >3/8 inch in depth or any twist visible to the naked eye requires immediate load removal and engineering evaluation?
- Baseplates inspected: each baseplate flat and fully in contact with floor; anchor bolts present, tight, and undamaged; no baseplate lifted, cracked, or showing corrosion that reduces effective bearing area?
- Out-of-plumb measurement taken for each upright: uprights leaning more than 1/2 inch per 10 feet of height in any direction tagged 'Do Not Load' pending engineering assessment?
- Impact-damaged uprights tagged with high-visibility 'Damaged - Do Not Load' tag; rack zone barricaded pending repair; all adjacent bays in the same row inspected for secondary damage from the same impact event?
Beam & Beam Connector Integrity
ANSI MH16.1-2021 §8.4 requires beam and connector inspection as part of the routine rack safety programme. Beam step connectors (safety clips or locking pins) prevent beams from being dislodged by upward forklift contact and are a required safety feature under ANSI MH16.1.
- Each beam inspected for visible deflection: center-span deflection exceeding span/180 (e.g., >1/3 inch for a 60-inch beam) or any visible permanent set requires immediate unloading and beam replacement?
- Beam step connectors (safety clips or locking pins) present and fully engaged at every connector location; missing or partially engaged connectors replaced before the beam is considered serviceable?
- Beam levels inspected for uniformity: all beams in a bay at each level must be at the same elevation; significant height difference between beam pairs at the same level indicates connector failure or upright damage?
Load Rating, Posted Capacity & Configuration Compliance
ANSI MH16.1-2021 §1.4 and §8.5 require that the load application and rack configuration (LARC) be posted conspicuously on the rack, and that actual loading not exceed the design capacity. Capacity placards are not optional - they are a required safety device.

Aisle Clearance, Column Protectors & Forklift Interface
OSHA 29 CFR 1910.22 requires that aisles be adequate in width for safe operation of equipment. OSHA 29 CFR 1910.178(e) requires adequate lighting and aisle marking for powered industrial truck (forklift) operation. ANSI MH16.1-2021 §8.3 requires column protection at locations subject to vehicle impact.
- Aisle widths measured and confirmed to meet forklift operating manual minimums for the equipment used in each aisle; aisle boundaries marked (painted lines or physical barriers) and not encroached by product or equipment?
- Column protectors (upright guards) installed at all end-of-aisle uprights and at any upright within 18 inches of a forklift travel path; guards in good condition (not themselves damaged to the point of being pressed against the upright)?
Floor Anchoring, Cross-Bracing & Seismic Considerations
ANSI MH16.1-2021 §8.3 and §9 require that floor anchors be installed as designed and that cross-bracing be present and undamaged. In seismic zones, ANSI MH16.1-2021 Appendix A requires that rack systems be designed for applicable seismic forces.
- All floor anchors verified present (not removed for flooring repair and left out), tight, and not showing pull-out cracking in the surrounding concrete; anchors in deteriorated concrete flagged for re-anchoring evaluation?
- Cross-aisle bracing and column bracing verified present at all required locations per rack installation drawings; bent or missing bracing members identified and rack unloaded pending repair?
Inspection Programme, Repair Records & Training Documentation
ANSI MH16.1-2021 §8.4 requires a documented rack inspection programme. OSHA CPL 02-01-047 (Powered Industrial Trucks) and General Duty Clause enforcement establishes that employers must maintain records of rack inspections and repairs. Undocumented rack inspection is equivalent to no inspection in an enforcement context.
