Scaffolding Inspection Checklist
Complete this comprehensive inspection before each use of scaffolding per OSHA 29 CFR 1926.451-454, ANSI A10.8, and international standards including UK HSE TG20:13, India IS 2750, Brazil NR-18, and Mexico NOM-009-STPS-2011. Do not use scaffold if any critical items fail inspection. Only competent persons may inspect scaffolds.

Foundation & Base Stability
Inspect base plates, mud sills, screw jacks, and foundation stability per OSHA 1926.451(c)(2). Base must be on firm footing capable of supporting scaffold and workers.

- Mud sills adequate size for soil conditions?
- Foundation firm, level, and capable of supporting loads?
- Screw jacks properly adjusted and fully engaged?
- No undermining or erosion of foundation?
- Casters locked (mobile scaffolds)?
Structural Frame & Connections
Inspect frames, bracing, and connections per OSHA 1926.451(a) and manufacturer specifications. All components must be in good condition.
- Frames plumb within manufacturer tolerance?
- Frames level at each lift?
- All cross braces installed and secured?
- Horizontal braces at proper intervals?
- Coupling pins and spring clips properly installed?
- All components free of bends, cracks, corrosion, damage?
- All components from same manufacturer/system?
Ties to Structure & Stability
Inspect ties, guys, and braces connecting scaffold to building per OSHA 1926.451(c)(1). Proper tie-ins prevent scaffold overturn.

- All tie connections secure to structure?
- Ties adequate strength for scaffold loads?
- Guy wires properly tensioned (if used)?
- Outriggers installed where required?
Platforms & Planking
Inspect work platforms, planking, and decking per OSHA 1926.451(b). Platforms must be at least 18 inches wide and fully decked.
- Platform width minimum 18 inches (20 inches for debris net)?
- Platforms fully decked between guardrails?
- Planks cleated or secured against movement?
- Plank overhang 6-12 inches (unless restrained)?
- Planks free of cracks, splits, knots, and damage?
- Scaffold-grade planks (or equivalent load rating)?
- Gaps between planks less than 1 inch?
Guardrails & Toeboards
Inspect fall protection guardrails per OSHA 1926.451(g). Top rails at 38-45 inches, mid-rails, and toeboards required on platforms 10+ feet.

- Top rail withstands 200 lbs downward/outward force?
- Mid-rail installed approximately halfway?
- Mid-rail withstands 150 lbs force?
- Toeboards installed (4 inches minimum)?
- Toeboard gaps less than 1/4 inch?
- Guardrails on all open sides?
Access & Egress
Verify safe access to scaffold platforms per OSHA 1926.451(e). Proper ladders, stairs, or ramps must be provided.

Load Capacity & Posted Limits
Verify scaffold load rating and ensure not overloaded per OSHA 1926.451(a)(1) and manufacturer specifications.

- Materials stacked orderly and not blocking access?
- Platform clear of debris, ice, and slipping hazards?
Electrical Hazards
Check clearance from power lines per OSHA 1926.451(f)(6). Maintain safe distances from energized conductors.
- Scaffold at least 10 feet from power lines <50kV?
- Clearance increased for higher voltages?
- Insulated barriers in place if minimum clearance not possible?
- GFCI protection for electrical tools on scaffold?
Training & Documentation
Verify worker training per OSHA 1926.454 and documentation requirements.
- All scaffold users trained per OSHA 1926.454?
- Scaffold erectors trained in assembly/disassembly?
- Competent person available during scaffold use?
- Scaffold tagged with status (Green/Yellow/Red)?
- Previous inspections documented and available?
