Shut down and isolate all energy sources.
- Equipment shut down using normal procedures?
- Electrical energy isolated?
- Hydraulic energy isolated and bled?
- Pneumatic energy isolated and bled?
- Mechanical energy blocked/released?
- Thermal energy addressed?
- All stored energy dissipated/restrained?
Lockout/Tagout Application
Apply locks and tags.
- Lock(s) applied to each isolation point?
- Tags attached with required information?
- Group lockout box used (if applicable)?
- All isolation points secured?
Energy Isolation Verification
Verify zero energy state.
- Tried operating controls to verify isolation?
- Controls returned to off/neutral?
- Voltage/energy tested with appropriate meter?
- Zero energy state confirmed?
Restoration (After Work Complete)
Safe removal of LOTO and restart.
- Work completed?
- All tools and materials removed?
- Machine guards replaced?
- All employees clear of equipment?
- Locks removed by authorized employee only?
- Affected employees notified of restart?
- Equipment tested and operating normally?
