Lockout Application
OSHA 1910.147(d)(4) lockout/tagout device application.

- Main disconnect switch opened/circuit breaker tripped?
- Personal lock applied to energy isolating device?
- Lockout tag attached with name, date, and reason?
- If multiple workers, each applied their own lock (group lockout)?

Zero Energy Verification
OSHA 1910.147(d)(6) verification of isolation effectiveness.
- Voltage meter tested on known live source first?
- Zero voltage verified at all phases and ground?
- Meter re-tested on known live source after verification?
- Attempt to start equipment at normal controls (if safe)?
- All downstream equipment verified de-energized?
- Capacitors discharged and grounded (if present)?
Restoration Procedure
OSHA 1910.147(e) release from lockout/tagout.
